What is a breast augmentation with an implant?
Breast augmentation is one of the most popular and most frequently requested cosmetic operations in Germany. At our practice clinic in Oldenburg we specialise in breast aesthetics and offer you modern methods of breast augmentation. Over the past four decades the silicone breast implant has proved itself as a well-established method.
What matters before any breast procedure is that you can describe your own wish clearly. Questions you should ask yourself: do I want a noticeably larger or a better defined breast? Should my breast look natural, like a teardrop, or rather round? Do I want a moderate or a strongly projected breast?
What can I expect from a breast augmentation with an implant?
Depending on the shape of your chest and the “footprint” of the breast on the chest, we select the implant that suits you – taking your wishes into account. We can do this as early as the first consultation. For particularly athletic women or for particularly large implants there are lighter B-Lite implants from the company Polytech: air bubbles introduced into the cohesive gel save about 30 % of the weight. We use exclusively German brand implants.
We favour access via the inframammary fold: it is safe, reliable and inconspicuous at the same time. In the event of a later implant exchange this access would be chosen anyway. Because every body brings different conditions with it, we also plan the position of the implant individually with you. If there is additional excess skin, the augmentation can be combined with a breast lift – using the O-incision technique around the nipple or as a lollipop lift with an additional vertical incision.
We are also happy to advise you on hybrid breast augmentation: a combination of autologous fat plus an implant or of locally transferred fatty tissue from the armpit plus an implant.
How long does an implant last?
The implants used today show hardly any material fatigue. Reasons for an exchange are usually the formation of a capsule (capsular fibrosis) and changes to the body above the implant through ageing, weight fluctuations or pregnancy.
When does an implant have to be exchanged?
The average time a breast implant stays in place in Germany is about 12 years. If capsular fibrosis with symptoms occurs, the implant should be exchanged. We occasionally see patients who have had an implant for 20 years with a result that is still beautiful. If no effusion and no damage to the implant can be shown on the ultrasound check, an implant can continue to remain in place.
How many millilitres for a cup size?
If we want to enlarge the breast by one cup size, that is about 180 ml of implant volume per breast.
Are there alternatives to breast implants?
The breast can also be enlarged with autologous fat. The fat is introduced in a fan shape into the pectoral muscle and the deep layers of the skin and enlarges the breast by about half a cup size to one cup size per session. During a lift we prefer to use local glandular tissue instead of implants as an autoaugmentation – an “implant” made of your own body tissue, so that you are spared a foreign body.
How does a breast augmentation with an implant work at our clinic?
As early as the preparatory consultation with advice, detailed information about risks and an examination, we plan the operation together. Please feel free to bring a loose bra in the size you want: we insert so-called test implants so that you can picture the result better and so that we can determine the right shape together. Our aim is to respond to your wishes as well as possible before the operation and to explain the course of the incision.
On the day of the operation, which we carry out in our own rooms in Oldenburg, you are scheduled in such a way that long waiting times are avoided. I will then go through what we discussed with you once more and carry out the markings. Our nursing and anaesthesia team will look after you. After the procedure you stay for one night in one of our patient beds; a nurse looks after you throughout the night. I myself am close by and take personal care of your physical wellbeing. After the final ward round the following day you can usually leave our clinic. We do not normally use drains for this procedure.
Advantages, disadvantages and risks of a breast augmentation with an implant
With the insertion of the implant the desired size is reached immediately; the method is quick and carries little risk. As with every operation, bruising, infection or wound healing problems are possible. On average an implant exchange is likely after 10 to 15 years, because the internal scarring around the implant (capsular fibrosis) can increase. Good alternatives that we offer are breast augmentation with autologous fat and hybrid breast augmentation.
What should be considered after a breast augmentation with an implant?
- A compression bra and a specially supporting breast band are worn for up to 6 weeks after the procedure in order to achieve the best possible result.
- No stitches need to be removed when resorbable suture material is used.
- You are usually able to face other people and to work again after 3 to 7 days.
- Sport may be started again carefully after 6 weeks; showering is possible from the 2nd day after the operation.
- Check-up appointments take place after 2 and 6 weeks.
